Online Application: Dialectical Behavior Therapy Intensive Training™

Thank you so much for your interest in our Dialectical Behavior Therapy Intensive Training™. As part of the application process we require submission of the following:

  1. Team Application (to be completed by the designated Team Leader)
  2. Individual Application (for each Team Member including the Team Leader)

The completion of both applications are required to review the Team’s eligibility to attend the DBT Intensive Training.

Application Steps:

  • STEP 1: Please set aside at least 20-30 minutes in order to complete the application.
  • STEP 2: Find your Application Fee Order # when you paid for your Application Fee. Submitting an Application Fee reserves your spot for the training. You should have received the 5-digit Order Number via email. It is also saved in the order section of your Behavioral Tech account on our website.
  • STEP 3: Please make sure you have the following required information in order to submit the application. You may not complete this application without providing the information below. Your application will not be saved, so any information entered will be lost if you leave the application prior to full completion.
    • Team Application:
      • Your DBT Consultation Team name
      • List of Team Members and their Role(s) in the Team
      • When and how often your DBT Team meets
      • Team/Program History
      • Organizational/Program Information & Population Type
    • Individual Application:
      • Your DBT Team name
      • Your DBT Training & Experience (including workshops, articles/books you have read, etc.)
      • When and how often your DBT Team meets
      • Confirmation that you have reviewed and agreed to all terms of the Training Agreement
      • Full CE Credentials and Contact information

Please remember to set aside 20-30 minutes to complete the application.

What happens after your submit your application?

We will review and process your application within 5-10 business days. We will contact you via email to confirm your acceptance.
